This most recently occurred on August 28, 2019 on the jobsite located at2106 S. 84th St., Omaha, NE 68124. The owner and 1 employee were observed installing shingles on an existing 2-story house without any form fall protection. Height from the ground to the edge of the roof is approximately 17 feet and the roof has a 4 in 12 pitch.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: The employer is failing to ensure that all employees are protected from fall hazards when entering and exiting the landing surface of a residential roof. This most recently occurred on August 28, 2019 on the jobsite located at2106 S. 84th St., Omaha, NE 68124. The owner and 1 employee were using an aluminum extension ladder to enter and exit the roof where the side rails were not extended to at least 3 feet above the landing surface which is about 10 feet above the ground.
