29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling:  (a)United States Department of Veteran Affairs Fort Meade VA Medical Center in the boiler plant at 113 Commanche Road, Fort Meade, SD 57741: On or about and at times prior to August 27, 2019, the employer did not ensure  employees performing work on work platforms on boilers were protected from fall hazards in that the work platforms did not provide appropriate fall protection. This condition exposed employees to fall hazards greater than 4 feet.

29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice:  (a)United States Department of Veteran Affairs Fort Meade VA Medical Center in Building 146 at 113 Commanche Road, Fort Meade, SD 57741: On or about and at times prior to August 27, 2019, the employer did not ensure employees were protected from tripping hazards in that the flexible electrical cord that was used to provide power to the Movincool Office Pro 60 in the Nutrition and Food Area protruded from the floor and ran along a walking working surface used by employees. This condition exposed employees to tripping hazards.

29 CFR 1910.305(a)(2)(x): The employer did not ensure that flexible cords and cables were protected from accidental damage:  (a)United States Department of Veteran Affairs Fort Meade VA Medical Center in Building 146 at 113 Commanche Road, Fort Meade, SD 57741: On or about and at times prior to August 27, 2019, the employer did not ensure employees were protected from electrical hazards in that the flexible electrical cord that was used to provide power to the Movincool Office Pro 60 in the Nutrition and Food Area protruded from the floor and ran along a walking working surface used by employees and was not protected from accidental damage. This condition exposed employees to electrical hazards.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:  (a)United States Department of Veteran Affairs Fort Meade VA Medical Center in the boiler plant at 113 Commanche Road, Fort Meade, SD 57741: On or about and at times prior to August 27, 2019, the employer did not ensure employees performing work with or in the vicinity of in-line storage containers of RLT-89, BWT-18 and BWT-284 were protected from chemical hazards in that the containers were not labeled to provide at least general information regarding the hazards of the chemicals. This condition exposed employees to chemical hazards.
