29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights is no more than one inch wide:  On or about August 27, 2019, the platform on the third working level of the scaffold used for residential plastering was not fully planked. This condition exposed employees to the hazard of falling to the ground below.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:  On or about August 27, 2019, the scaffold used to apply plaster to the home was resting only on the scaffold frame. This condition exposed employees to fall hazards during scaffold collapse.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  On or about August 27, 2019, plastering employees accessed the scaffold via climbing an extension ladder in front of the house and descending a chicken ladder to the scaffold. This condition exposed employees to the hazard of falling from the ground below.

On or about August 27, 2019, crawling boards used by employees for scaffold access were not secured to the roof via ridge hooks or other similar means. This condition exposed employees to the hazard of falling to the ground below.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  On or about August 27, 2019, employees plastering the walls were using a scaffold without fall protection at least 11.6 feet above the ground below. This condition exposed employees to the hazard of falling to the ground below.

29 CFR 1926.451(h)(2)(ii): A toeboard was not erected along the edge of platforms more than 10 feet (3.1 m) above lower levels for a distance sufficient to protect employees below:Note:  This requirement does not apply to float (ship) scaffolds where an edging of 3/4 inch x 1 1/2 inch (2 x 4 cm) wood or equivalent was used in lieu of toeboards.  On or about August 27, 2019, the scaffold used for plastering was not built with toeboards. This condition exposed employees to struck by hazards.
