29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about August 22, 2019, at 2600 N. 64th Avenue, Hollywood, Florida 33024, an employee was exposed to an amputation hazard while repairing a water surge relief valve without a complete energy control program being established for the type of work function. NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section: On or about August 22, 2019, at 2600 N. 64th Avenue, Hollywood, Florida 33024, an employee was performing repairs on a surge relief valve, during which the employer had not developed and implemented energy control procedures to clearly and specifically outline the scope, purpose, authorization, rules and techniques to be utilized in the control of hazardous energy for the surge relief valve spring and the means to enforce compliance; thereby exposing the employee to an amputation hazard. NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
