29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:    Employee(s) were exposed to trip, fall and engulfment hazards.  At the jobsite located at 2947 189th Terrace in Gardner, Kansas, an employee in an excavation installing a protection boot on an underground utility line at approximately nine feet  in depth was exposed to trip, fall, and cave in hazards.  A safe means of egress was not provided in the excavation.

At the jobsite located at 2947 189th Terrace in Gardner, Kansas, an employee in an excavation installing a protection boot on an underground utility line at approximately nine feet  in depth was exposed to cave-in hazards.  A protective system was not used at the west ene of the excavation.
