At the jobsite located at 2947 189th Terrace in Gardner, Kansas, an employee in an excavation installing a protection boot on an underground utility line at approximately nine feet in depth was exposed to trip, fall, and cave in hazards. A safe means of egress was not provided in the excavation.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or 1926.652(c): Employee(s) were exposed to struck-by and engulfment hazards. At the jobsite located at 2947 189th Terrace in Gardner, Kansas, an employee in an excavation installing a protection boot on an underground utility line at approximately nine feet in depth was exposed to cave-in hazards. A protective system was not used at the west ene of the excavation.
