29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) 800 High Street - The employer did not ensure that access to the E-turn tube bending machine - Asset 1420, was adequately guarded to prevent unauthorized access during machine operations. The front access to the machine was protected by light curtains, however both the left, right and rear were only protected by chain,  which could be removed, on or about August 29 2019.    b) 800 High Street - The employer did not ensure that the pressure sensing mats for four Bender machines - Assets 1423, 8211, 8214 and 1417, were placed in a position that provided adequate guarding by distance when the machine was in operation on or about August 29, 2019.     c) 800 High Street - The employer did not ensure that 17 polishing wheels located in the inner and outer polishing rooms were properly guarded to prevent the operator from hazards including but not limited to: struck by, caught in or rotating parts, on or before August 29, 2019.    d) 800 High Street - The employer did not ensure that two belt sanding machines located in the inner and outer polishing rooms were properly guarded to prevent the operator from hazards including but not limited to: struck by, caught in or rotating parts, on or before August 29, 2019.    e) 800 High Street - The employer did not ensure that the large hexagonal nuts on the ends of the 17 polishing wheels and two belt sanding machines were properly covered to prevent employees from becoming caught in or struck by the hexagonal nuts, on or before August 29, 2019.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:    a) 800 High Street -  The work rest on the Porter Cable Ball Bearing Grinder Model 547 used to deburr tubing exceeded the maximum 1/8 inch requirement and had a distance of 5/8 of an inch away from the wheel, on or about August 29, 2019.    No abatement certification or documentation required.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  a) 800 High Street - The tongue guard Baldor grinder/buffer P0104 on the eight inch bench grinder used to deburr tubing exceeded 1/4 inch requirement and had a distance of 7/8 of an inch, on or about August 29, 2019.  No abatement certification or documentation required.
