29 CFR 1910.147(d)(3): All energy isolating devices that are needed to control the energy to the machine or equipment was not physically located and operated in such a manner as to isolate the machine or equipment from the energy sources:  On or about August 13, 2019 the employer failed to ensure that the hazardous energies of the 770 ton horizontal injection molding machine were located and isolated to eliminate the unexpected start up and release of the energies. Employees routinely reach over the front interlocked door during operation to blow off and wipe condensation and water from the equipment. This task is performed without the use of LO/TO.
