Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: industrial storage rack collapse and struck by hazards:  On or about 8/29/2019 in the warehouse area workers on foot and or operating powered industrial equipment were exposed to struck by and crushing hazards where the loaded industrial storage racks were visibly damaged by powered industrial trucks at locations including but not limited to:  a. D28 - stretcher bent b. E28 - stretcher bent c. G27 - stretcher bent d. G13 - damaged upright e. H34 - stretcher bent f. I34 - stretcher bent g. J16 - stretcher bent h. K15 - stretcher bent i. L33 - stretcher bent j. L33 - shelf bent (unloaded/available for use) k. L16 - stretcher bent l. M16 - stretcher bent m. N16 - stretcher bent n. N16 - stretcher bent o. Q7 - stretcher bent p. Q8 - shelf bent (loaded) q. Q13 - stretcher bent r. Q18 - stretcher bent s. Q17 - stretcher bent t. R25 - stretcher bent u. R8 - stretcher bent v. S15 - stretcher bent w. T16 - stretcher bent   One feasible means of abatement would be to maintain the structural integrity of the installed rack system by assuring proper operational, housekeeping, and maintenance procedures including but not limited to: Regularly inspect for damage. If damage is found, IMMEDIATELY unload the affected area and replace or repair any damaged columns, beams, or other structural components."

29 CFR 1910.157(e)(2): Portable fire extinguishers were not visually inspected at least monthly:  a. On or about 8/29/2019, throughout the facility workers expected to fight incipient level fires were exposed to smoke inhalation and burn hazards where the employer failed to inspect the portable fire extinguishers since installed in 12/17/2018.   b. On or about 8/29/2019, there was an extinguisher mounted to the Linde E25C powered industrial truck that was installed on 7/24/2018 that was not inspected monthly.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a. On or about 8/29/2019, at the Validation Shipping area #2, employees were observed exposed to electrical hazards where the electrical service did not meet listed labeling where the 15 A relocatable power tap (RPT) powered a 1 A phone charger/speaker cord, an 11 A printer, an 8 A Bosch battery charger, a 2 A internet router, a .3 A computer monitor, and a red extension cord powering a 1.2 A scale. Additionally, portable heaters were also powered by the same RPT.  b. On or about 8/29/2019, at the Picking shipment area #2, an employee was observed exposed to electrical hazards where the electrical service did not meet listed labeling where the 15 A relocatable power tap (RPT), powered by the green extension cord, powered an orange extension cord powering a .8 A Lasko box fan, a 500mA scale, a 500mA scale, and a 13 A Ryobi 7 3/4-inch circular saw S/N CSB125. Additionally, portable heaters were also powered by the same RPT.  c. On or about 8/29/2019, at the shipping workstation, workers were exposed to electrical hazards where the metallic, quad outlet was attached to a flexible cord being used as a pendant.  d. On or about 8/29/2019, at the shipping workstation, workers were exposed to electrical hazards where the metallic, quad outlet powered a relocatable power tap (RPT) that powered a red extension cord that powered another RPT powering the bubble wrap machine. The metallic, quad outlet also powered an RPT that powered an RPT powering a personal fan, a printer, a printer, and a monitor. The metallic, quad outlet also powered an internet router. Additionally, portable heaters were also powered by the same metallic, quad outlet.  e. On or about 8/29/2019, at the shipping workshop, workers were exposed to electrical hazards where the 15 A relocatable power tap (RPT) was powered by an orange extension cord. The RPT powered a 2 A internet router, a 1.5 A label printer, an 11 A Xerox printer, and a .3 A monitor. Additionally, portable heaters were also powered by the same RPT.

On or about 8/29/2019, at the Picking shipment area #2, where an employee was observed exposed to electrical hazards where the electrical service did not meet listed labeling where the 15 A relocatable power tap (RPT), powered by the zip tied green extension cord being used as permanent wiring, powered an orange extension cord powering a .8 A Lasko box fan, a 500mA scale, a 500mA scale, and a 13 A Ryobi 7 3/4-inch circular saw S/N CSB125. Additionally, at times where the weather was colder portable heaters were also powered by the same RPT.  b. On or about 8/29/2019, at the shipping workshop, workers were exposed to electrical hazards where the 15 A relocatable power tap (RPT) was powered by a zip tied orange extension cord being used as permanent wiring. The RPT powered a 2 A internet router, a 1.5 A label printer, an 11 A Xerox printer, and a .3 A monitor. Additionally, at times where the weather was colder portable heaters were also powered by the same RPT.  c. On or about 8/29/2019, at the shipping workstation, workers were exposed to electrical hazards where the metallic, quad outlet was attached to a flexible cord being used as permanent wiring.

On or about 8/29/2019, at the battery charging area, METEK 880H3-18C charger, S/N 215CS33495, where employees were exposed to electrical hazards where there was exposed copper on the DC (RED) cable.  b. On or about 8/29/2019, at the battery charging area, METEK 750H3-18C charger, S/N 215CS30948, where employees were exposed to electrical hazards where there was previously exposed copper on the DC (RED) cable, repaired with electricians tape.
