29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) Campa Roofing located at 713 Parsons Ave. Columbus, OH 43206: On and preceding August 29, 2019,  the employer did not ensure that employees engaged in residential construction activities 6 feet or more above lower level were protected from falls. Roofers were conducting removing shingles on a roof that varied from 10-24 feet with the employees working at different levels. The employees were not utilizing proper fall protection which exposed them to falls up to 24 feet.

29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:    a) Campa Roofing located at 713 Parsons Ave. Columbus, OH 43206: On and preceding August 29, 2019, employees were removing shingles, preparing for a re-roof on the roof of a two story residential home at heights up to 24 feet without fall protection. The employer did not initiate and maintain safety and health programs related to the roofing work being conducted by employees.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury    a) Campa Roofing located at 713 Parsons Ave. Columbus, OH 43206: On and preceding August 29, 2019, the employer did not instruct employees in the recognition and avoidance of unsafe conditions and the procedures to be followed in order to minimize these hazards. Roofers were removing/installing shingles on a roof that varied from 10-24 feet with the employees working at different levels. The employees were not utilizing proper eye, head or fall protection which exposed them to falls up to 24 feet and struck by hazards to the head and eyes.
