29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: An annual lockout/tagout inspection, including review of written procedures and review between the inspector and each authorized employee, had not been conducted within the required twelve month period. 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Guarding was not installed on the W-3 welding conveyor. Employees stood between the conveyor horizontal cross beams during the welding step. Upon completion of the welding step and when the conveyor was started to move product from weld conveyor to the quality control conveyor, employees were exposed to a struck by hazard from the moving cross beams. (Note: employees were exposed to the same struck by hazard as the product was transferred from the jig conveyor line to the weld line.)
