29 CFR 1926.501(b)(4): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  a) 325 S. 18th Street, Philadelphia, PA 19103 - 1st Floor: The employer does not ensure that employees were protected from falling through a basement/open stairway floor hole exposing employees to a 6 foot fall.   Condition observed on or about August 27, 2019.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) 325 S. 18th Street, Philadelphia, PA 19103 - Rooftop: The Employer did not provide fall protection for employees exposed to fall hazards in excess of 26 feet to the ground surface below while conducting deck installation on a residential roof.    Condition observed on or about August 27, 2019.    Webb Contractors, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1376138, citation number 1, item number 1 and was affirmed as a final order on 05/21/2019, with respect to a workplace located at 58 & 60 Bradhurst Ave., Lakewood, NJ 08701.  Webb Contractors, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1393968, citation number 1, item number 1 and was affirmed as a final order on 07/10/2019, with respect to a workplace located at 14 & 16 Buttell Ave., Lakewood, NJ 08701.  Webb Contractors, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1397826, citation number 1, item number 1 and was affirmed as a final order on 07/10/2019, with respect to a workplace located at 93 & 95 Broadway Ave., Lakewood, NJ 08701.

Condition observed on or about August 27, 2019.    Webb Contractors, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1393968, citation number 1, item number 2 and was affirmed as a final order on 08/10/15, with respect to a workplace located at 14 & 16 Buttell Ave., Lakewood, NJ 08701.
