On or about August 26, 2019 the employer failed to train employee(s) on the operation of forklifts exposing employees to struck by hazards. No documentation or certification is required.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about August 26, 2019 the employer did not ensure that the bag cutting knife and other components on the HMC 2 machine were guarded exposing employees to laceration hazards when the machine is running or when the guides are adjusted. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
