OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish the employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to crushing hazards:    a) Warehouse Building 2: Employees were exposed to four tier racks storing pallets of books that had damaged vertical uprights, cross braces and horizontal shelving in areas including, but not limited to, 201-003-50, 201-007-50, 207-019-50, 207-025-10, 207-055-10, 207-074-10, 210-006-10, 210-035-50, 210-042-10, 210-046-10, 210-056-10, 210-056-50, 210-85-10, 213-006-10, 219-024-42, 219-035-50, 219-041-10, 219-065-10, 219-079-50, 221-008-50, 221-009-10, 221-018-10, 221-066-10, 221-082-10, 223-004-50, 223-005-10, 223-014-10, 223-021-10, 298-024-05 and 298-038-10, on or about 9/4/2019.    b) Warehouse Building 3: Employees were exposed to four tier racks storing pallets of books that had damaged vertical uprights, cross braces and horizontal shelving in areas including, but not limited to, 301-89-50, 317-092-10 and 317-109-50, on or about 9/4/2019.    c) Warehouse Building 4: Employees were exposed to four tier racks storing pallets of books that had damaged vertical uprights, cross braces and horizontal shelving in areas including, but not limited to, 402-011-10, 402-015-10, 402-019-10, 409-022-10, 409-170-50 and 409-200-50, on or about 9/4/2019.    d) Warehouse Buildings 5: Employees were exposed to four tier racks storing pallets of books that had damaged vertical uprights, cross braces and horizontal shelving in areas including, but not limited to, 537-053-10, on or about 9/4/2019.      NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
