Campbell Rd., Richardson, TX 75081; where at least one (1) employee was standing on the toprail and midrail of a lift basket while performing the work.

Campbell Rd., Richardson, TX 75081; where at least one (1) employee was working from vehicle-mounted lift basket was not wearing a personal fall arrest or travel restraint system to protect him from falling to the ground below.
