Near granulator #3, left side, where employees were exposed to a fall hazard from tripping into or stepping into or through a hole less than 4 feet above a lower level without covers or guardrail systems. d. 29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws. On or about September 3, 2019: a. On granulator #1, right side, at the Jacob safety joy stick where employees were exposed to wiring showing at the strain relief. b. On granulator #2, left side, at the Jacob safety joy stick where employees were exposed to wiring showing at the strain relief. c. On granulator #3, left side, at the Jacob safety joy stick where employees were exposed to wiring showing at the strain relief. d. Near and between granulator #1 and #2, on the walkway railing, on the fan electrical cord where employees were exposed to wiring showing at the strain relief.
