29 CFR 1926.405(g)(2)(iii): Flexible cords were not used in continuous lengths without splice or tap:  a) On or about August 28, 2019, and at times prior thereto, the employer installed a makeshift power connection by splicing together vehicle jumper cables and a flexible cord and attaching to the outdoor air conditioning unit electrical bus bar. The assembly was used to power portable hand tools, exposing employees to an electrical shock hazard.  b) On or about August 28, 2019, and in times prior thereto, on the roof, excessively worn cables and cords were spliced with wire nuts, without insulating materials equivalent to the original cords.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  The employer did not provide a training program for all employees exposed to fall hazards.  The program shall include training and instruction in the following areas: a)  The recognition and identification of fall hazards in the work area; b)  The use and operation of guardrail systems (including perimeter safety cable systems), personal fall arrest systems, positioning device systems, fall restraint systems, safety net systems, and other protection to be used; c)  The correct procedures for erecting, maintaining, disassembling, and inspecting the fall protection systems to be used; d)  The procedures to be followed to prevent falls to lower levels and through or into holes and openings in walking/working surfaces and walls; and e)  The fall protection requirements of this subpart.
