29 CFR 1926.20(b)(2): Such programs did not provide for frequent and regular inspections of the job site, materials and equipment to be made by competent persons designated by the employer: On or about August 28, 2019, and at times prior thereto, workers were replacing the siding on the chimneys of the apartment complex without the employer ensuring that frequent and regular inspections of the job site were conducted by a competent person, exposing employees to the hazards associated with their work.

29 CFR 1926.602(c)(1)(ii): Modifications or additions affecting the safe operation of the equipment were made without the manufacturer's written approval: Note: If such modifications or changes are made, the capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly: On or about August 28, 2019, and at times prior thereto, a wooden box was attached to the forklift for loading tools and materials to the roof without the manufacturer's written approval, exposing workers to being struck by the box.
