29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems:   (a)    Fourth Floor Framing - On or about August 28, 2019, structural steel workers, who were exposed to a 20 foot fall, were observed landing loads of steel decking and web joists lowered to them from a truck crane and were not using personal fall protection during that process.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.760(a)(2): Perimeter safety cables were not installed on final interior and exterior perimeters on the floors as soon as the metal decking was installed:   (a)    Second Floor Framing, South Side  On or about August 28, 2019, workers were exposed to a 10 foot fall for safety cables were not installed on the open side of a steel framed multi-story building where this contractor had completed decking the floor.   (b)   Second Floor Framing, West Side - On or about August 28, 2019, workers were exposed to a 10 foot fall for safety cables were not installed on the open side of a steel framed multi-story building where this contractor had completed decking the floor.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:   a)  Second Floor, Existing Building  On or about August 28, 2019, workers were exposed to a 10 foot fall when an 8 foot long portable step ladder was not used for the purpose it was intended, in that, the ladder sections were folded together and leaned up against a brick wall while it was used by workers to access their work area.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).   No abatement certification or documentation required for this item.
