29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by rotating and reciprocating parts:   Production Area:  On, or about, September 30, 2019, employees exposure to rotating and reciprocating parts was not minimized by an enclosure guard or other equally effective means in that the following Nilson fourslide machines were not provided with a two hand jog control for set-up:  A.  Green Four Slide, Model S-432. B.  Grey Four Slide, Model S-432.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    Production Area, Gray Nilson Fourslide Machine:  On, or about, September 30, 2019, a relocatable power tap was energized through a green extension cord for the handle dispenser.  The use of an extension cord to energize a relocatable power tap is not an acceptable use under the UL White Book which contains instructions for use of relocatable power taps.

29 CFR 1910.305(g)(1)(iv)(D): Flexible cords and cables were used where attached to building surfaces:    Production Area, Gray Nilson Fourslide Machine:  On, or about, September 30, 2019, the handle dispenser was energized through a series of two extension cords and a relocatable power tap.      The (employer name) was previously cited for a violation of an equivalent standard [29 CFR 1910.305(g)(1)(I)(A)], which was contained in OSHA Inspection Number 1007450, Citation Number 1, Item Number 5b and was affirmed as a Final Order on January 16, 2015, with respect to a workplace located at Torrington, Connecticut.

29 CFR 1910.217(e)(1)(i)(C):  The employer shall maintain a certification record of each inspection, and each maintenance and repair task performed, under the general component of the inspection program that includes the date of the inspection, maintenance, or repair work, the signature of the person who performed the inspection, maintenance, or repair work, and the serial number, or other identifier, of the power press inspected, maintained, and repaired:  Production Area:  On, or about, September 30, 2019, the employer had not maintained inspection records for the Precision Welder and Flexipress Corp. straight-sided partial revolution  mechanical power press.
