29 CFR 1926.651(b)(4): While the excavation was opened, underground installations were not protected, supported or removed as necessary to safeguard employees. On or about September 03, 2019, an employee was working inside a trench using a digging bar and did not protect the natural gas underground line. This condition exposed employees to fire and explosion hazards.
