29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: Jobsite located at Cargill Falls Mill, 58 Pomfret Street, Putnam, Connecticut: On, or about, September 3, 2019, three employees were performing demolition work in a multi-story structure. Two of the three employees were tied off to self-retracting lifelines that were deployed beyond the maximum working radius as determined by the manufacturer based on the height of the life-line and potential fall distance to the lower level. Cantos Carpentry and Construction Services LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA Inspection Number 1307205, Citation Number 1, Item Number 1a and was affirmed as a Final Order on July 18, 2018), with respect to a workplace located at New Haven, Connecticut.
