29 CFR 1926.201(a): Signaling by flaggers and the use of flaggers, including warning garments worn by flaggers did not conform to Part VI of the Manual on Uniform Traffic Control Devices, (1988 Edition, Revision 3 or the Millennium Edition), which are incorporated by reference in 1926.200(g)(2):  location: Roadway surrounding 1 Park Place  on or about: 8/30/19  a) Employees were signaling traffic during roadwork activities, such as concrete delivery. The employees were not using STOP/SLOW paddles. 29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  location: upper basement, building perimeter on or about: 9/24/19  a) An employee was erecting wooden guardrails around the building perimeter. The employee was not protected from falling 8'' 1", to the ground below.
