Located at 1765 Lancaster Newark Road in Lancaster, Ohio: On or about September 4, 2019, employees were tearing off an existing shingle roof and throwing the debris over an energized 220 Volt electrical service entrance, thereby exposing employees to an electrocution hazard. The employer did not assess the work site to determine if employees would be exposed to the electrical circuit during the course of the work and take protective measures.
