29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:    79 River Rd, Uxbridge, MA: On September 26, 2019 and at times prior, the company president operated a powered industrial truck that had a Titan FMJ3 crane attachment that was not listed on the data plate.  The PIT was regularly used to lift and move heavy sheets of marble and granite.  The PIT attachment reduced the lifting capacity and moved the load center far outside those listed on the data plate.
