29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about August 29, 2019, employees engaged in roofing activities were exposed to fall hazards of approximately 12ft. and 20ft. on a commercial construction site in Georgetown, Illinois, and fall protection was not being used on a steep roof.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On or about August 29, 2019, employees were exposed to a fall hazard, while accessing and exiting the work area via an extension ladder on a commercial construction site in Georgetown, Illinois, and the employer did not ensure the extension ladder extended at least 3 feet above the upper landing surface.
