29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed:  a) Dollar Tree Store #1557, Stockroom Exit  On or about September 5, 2019, employees worked throughout the stockroom to stock boxes/products, but excess boxes obstructed the stock room exit route.    b) Dollar Tree Store #1557, Sales Floor Rear Exit:  On or about September 5, 2019, employees worked throughout the sales floor for retail sales and customer assistance, but the sales floor rear exit route was obstructed with sales fixtures, shopping carts, and other material stored along the walls.    Dollar Tree Stores, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.37(a)(3), which was contained in OSHA inspection number 1309164, Citation 1 Item Number 1 and was affirmed as a final order on November 7, 2018, with respect to a workplace located at 1605 Morrissey Dr. Suite 106  Bloomington, IL. 61704.    Dollar Tree Stores, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.37(a)(3), which was contained in OSHA inspection number 1293367, Citation 1 Item Number 1 and was affirmed as a final order on August 20, 2018, with respect to a workplace located at 9208 East R L Thorton Dallas TX. 75228. 29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:  a) Dollar Tree Store #1557: On or about September 5, 2019, employees worked behind the counter where the balloon filling station and cash register were located, but the helium cylinders located in the area were not secured to prevent them from being knocked over.      b) Dollar Tree Store #1557: On or about September 5, 2019, employees worked in the Stockroom where helium cylinders are stored, but these cylinders were not secured to prevent them from being knocked over.    Abatement Note:  Among other feasible means, cylinders must be stored in accordance with the Compressed Gas Association Pamphlet P-1 which states, "Section 3.2.3: Never drop cylinders nor permit them to strike against each other or against other surfaces violently" and "Section 3.4.4: Before using cylinder, be sure it is properly supported to prevent it from being knocked over".    Dollar Tree Stores, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.101(b), which was contained in OSHA inspection number 1206871, Citation 1 Item Number 1 and was affirmed as a final order on July 11, 2017, with respect to a workplace located at 727 N Charles, Warrenburg, MO. 64093.
