Location: Ward St and Farrand St, Bloomfield, NJ a) The employer permitted the use od a deteriorated rope as a horizontal life line and the horizontal lifeline used was not installed and used under the supervision of a qualified person thereby exposing an employee installing plywood to a potential fall hazard of approximately 12 ft. to the ground below; on or about 8/19/19.

Location: Ward St and Farrand St, Bloomfield, NJ. a) Employer was erecting rebar on the 3rd floor and did not provide toeboards for the guardrail systems that were erected at the unprotected edges thereby exposing employees walking/working below to overhead/struck by hazards from falling objects; on or about 8/19/19.
