29 CFR 1926.652(a)(1):  Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c):      (a)   H3P LLC, at 0177 Overlook Drive, Breckenridge, CO 80424:  On August 21, 2019, the employer did not ensure that each employee conducting work in an excavation was protected from cave-in, in that work performed to uncover an existing water line and to make a tie-in to install a valve was done in the area of the trench that was approximately 6-9 feet deep, with a near vertical wall on one side, and lacked a protective system.  This condition exposed employees to an engulfment and/or struck-by hazard from trench excavation wall cave-in.
