29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about September 4, 2019, at the north side of the house being located at 24910 SW 112 Court, Homestead, FL 33030, an employee was exposed to a 9-foot fall hazard when the ladder being used to gain access to the work level was not extended 3 feet over the landing surface.
