29 CFR 1910.28(b)(1)(i): Each employee on a walking/working surface having an unprotected side or edge which was four feet (1.2m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a) On September 3, 2019, employees were exposed to the hazard of falling up to six stories while unloading and delivering home appliances through apartment balconies without fall protection.

29 CFR 1910.27(a): Scaffolds.  Scaffolds used in general industry must meet the requirements in 29 CFR part 1926, subpart L (Scaffolds).    29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level:     a) On September 3, 2019, employees were exposed to the hazard of falling up to approximately six stories while working from a MEC Model 6092RT scissor lift which was loaded in such a manner that the rear gate could not close.
