The door had a one and a half hour fire rating. Violation was observed on November 22, 2019. b) Worksite was located at 385 Tremont Avenue; East Orange, NJ 07018 Employees were exposed to smoke inhalation and burn hazards as the fire door for the paint locker in the Paint Shop of Building 6 was propped open disabling the passive fire safety system in that area. The door had a three hour fire rating. Violation was observed on November 22, 2019.

Violation was observed on September 4, 2019. c) Worksite was located at 385 Tremont Avenue; East Orange, NJ 07018 Employees were exposed to electrical hazards as they were utilizing a Power Flite Fan with a missing ground pin in the Housekeeping Department from Room 5 - 256 on the 5th Floor of the Main Hospital to dry floors and clear air. Violation was observed on December 4, 2019. d) Worksite was located at 385 Tremont Avenue; East Orange, NJ 07018 Employees were exposed to electrical hazards as they were utilizing a Nilfisk Advance VL500 75-1 EDF US Wet/Dry Vacuum, Serial Number 3510171908960, with a missing ground pin in the Housekeeping Department from Room 3- 401 on the 3rd Floor of the Main Hospital to clean floors. The cord had burn marks on it from overheating. Violation was observed on December 4, 2019. e) Worksite was located at 385 Tremont Avenue; East Orange, NJ 07018 Employees were exposed to electrical hazards as they were utilizing a Nilfisk Advance VL500 75-1 EDF US Wet/Dry Vacuum, Serial Number 3510171908961, with a missing ground pin in the Housekeeping Department from Room 2 - 326 on the 3rd Floor of the Main Hospital to clean floors. Violation was observed on December 4, 2019. f) Worksite was located at 385 Tremont Avenue; East Orange, NJ 07018 Employees were exposed to electrical hazards as they were utilizing a work station in the area of Rooms 1 - 175 and 1 - 176 that had a burnt power blade. Violation was observed on December 4, 2019. g) Worksite was located at 385 Tremont Avenue; East Orange, NJ 07018 Employees in Building 5a were exposed to electrical hazards as they were utilizing a Nilfisk Advance VU500 Vacuum with had a power cord that had a damaged strain relief where the inner conductor wires were exposed and not protected by the outer insulation. Violation was observed on December 4, 2019. h) Worksite was located at 385 Tremont Avenue; East Orange, NJ 07018 Employees in Building 1 were exposed to electrical hazards as they were utilizing an AirKing High Velocity Air Circulator with had a power cord that had a missing ground pin. Violation was observed on December 4, 2019. i) Worksite was located at 385 Tremont Avenue; East Orange, NJ 07018 Employees in Building 18a and 18b were exposed to electrical hazards as they were utilizing a Nilfisk Advance VU500 Vacuum with had a power cord that had a damaged power cord where the inner conductor wires were exposed and not protected by the outer insulation.
