On or about September 5, 2019, at the jobsite, untrained employees were working in an excavation that was 6 feet deep, in type C soil, with vertical side walls, and cave-in protection was not being used.

On or about September 5, 2019, at the jobsite, untrained employees were working in an excavation that was 6 feet deep, 20 feet wide, and the extension ladder used for egress extended 2 feet past the landing and was 35 feet from the exposed employees.

On or about September 5, 2019, at the jobsite, the extension ladder used by employees, for egress, while working in a 6 foot deep excavation was 35 feet from the location of the exposed employees.
