29 CFR 1910.303(b)(6): Conductors or equipment were located in damp or wet locations; where exposed to gases, fumes, vapors, liquids, or other agents that had a deteriorating effect on the conductors or equipment; or where exposed to excessive temperatures and were not identified for use in the operating environment: Nitric Acid dip tank room, 102 Cabot Street, Holyoke, MA: On or about September 5, 2019 employees were using equipment such as but not limited to light switches, pumps, and flexible cords connected to receptacles which were located in a damp location and were exposed to gasses and vapors from Nitric acid. Nitric Acid vapors have deteriorating effects on both conductors and electrical equipment had not been not identified for use in such an environment.

29 CFR 1910.305(e)(2): Switches, circuit breakers and switchboards installed in wet locations were not enclosed in weatherproof enclosures: Throughout facility, 102 Cabot Street, Holyoke, MA: On or about September 5, 2019 employees were using electrical equipment such as but not limited to submersible pumps and fans which were connected to receptacles above fluid tanks and the receptacles were not enclosed in waterproof enclosures.
