Location: a) 2nd floor, top of stairway. a) On or about 09/05/2019, Access from the 1st floor to 2nd floor blocked by two wooden planks . Employees used the planks on the second floor stairway landing as a temporary elevated work platform which blocked only means of access up and down the stairs.
