29 CFR 1926.20(b)(1): The employer did not initiate and maintain programs necessary to ensure compliance with the Occupational Safety and Health Standards: 33100 County Rd. 31, NCYRTC Job Site: A safety and health program encompassing safety policies, a hazard assessment, and work rules, was not developed and/or implemented at the job site. On August 16, 2019 an employee fell approximately six feet while standing on the outside corner of a two-tier scaffold and sustained serious injuries due to the lack of safety and health programs and safety training.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury: 33100 County Rd. 31, NCYRTC Job Site: An employee was not instructed in the recognition and avoidance of unsafe conditions when he fell approximately six feet while standing on the outside corner of a two-tier scaffold. The employee sustained broken bones.
