29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  a)  For Citation 1, Item 1, issued on 11/8/2019 with an abatement date of 12/9/2019, the employer did not submit documents demonstrating the complete abatement of the cited violation to OSHA. b)  For Citation 1, Item 2, issued on 11/8/2019 with an abatement date of 12/9/2019, the employer did not submit documents demonstrating the complete abatement of the cited violation to OSHA. c)  For Citation 1, Item 3, issued on 11/8/2019 with an abatement date of 12/9/2019, the employer did not submit documents demonstrating the complete abatement of the cited violation to OSHA. d)  For Citation 2, Item 1, issued on 11/8/2019 with an abatement date of 12/9/2019, the employer did not submit documents demonstrating the complete abatement of the cited violation to OSHA. e)  For Citation 2, Item 2, issued on 11/8/2019 with an abatement date of 12/9/2019, the employer did not submit documents demonstrating the complete abatement of the cited violation to OSHA.

29 CFR 1926.502(d)(6)(iii): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any object which is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself:  Employees were utilizing vertical lifelines whose snaphooks were connected to an anchor point dee-ring that had another snaphook attached to it when employees were performing roofing operations at a height of approximately twenty (20) feet above the ground.

Hazaly Custom Building LLC was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1405589, citation number 1, item number 1B and was affirmed as a final order on 7/9/2019, with respect to a workplace located at 412 Michigan St., Westville, IL 61832.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-sloped roofs with unprotected sides and edges 6 feet (1.8m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line systems and guardrail systems, warning line systems and safety net systems, or warning line systems and personal fall arrest systems, or warning line systems and safety monitoring systems:    The employer did not ensure that fall protection was used when:  A.  Employees were moving shingles to the roof of the structure and exposed to the hazard of falling up to twenty (20) feet.  B.  Employees were engaged in commercial roofing operations and exposed to the hazard of falling twenty (20) feet.   Hazaly Custom Building LLC was previously cited for a violation of an equivalent occupational safety and health standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1405589, citation number 1, item number 2 and was affirmed as a final order on 7/9/2019, with respect to a workplace located at 412 Michigan St., Westville, IL 61832.
