OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to severe burn, traumatic injuries, and/or death due to the reaction of water/steam coming in contact with molten metal: (a) Facility, Casting Department: On or about September 7, 2019, four employees were exposed to severe burns due to the reaction of water/steam and molten metal when the hose fittings installed on the Continental ARC16SC-12 hose, were not installed properly. It was found that the fittings on the hose were not in accordance to manufacturer's specifications. The manufacturer recommended crimp on the hose is 2.72 and this fitting was measured at 1.35. On September 7, 2019, the ARC15SC-12 hose failed under pressure at the coupling and caused the hydraulic ram to drop thus allowing molten coper to come in contact with the water/steam mixture resulting in a violent molten metal explosion.
