29 CFR 1960.72: The agency did not submit to the Secretary by May 1 of each year all information included on the agency's previous calendar year's occupational injury and illness recordkeeping forms. The information submitted did not include all data entered on the OSHA Form 300, Log of Work-Related Injuries and Illnesses (or equivalent); OSHA Form 301, Injury and Illness Incident Report (or equivalent); and OSHA Form 300A, Summary of Work-Related Injuries and Illnesses (or equivalent): a. On or about 1/31/2020, at 7305 N. Military Trail, West Palm Beach, Florida, 33410, the OSHA 300 omitted the following requirements from block (F) such as, but not limited to, injury of illnesses, part of body affected, and object or substance causing the injury/illness. b. On or about 1/31/2020, at 7305 N. Military Trail, West Palm Beach, Florida, 33410, the OSHA 300A omitted accurate calculations of number of cases, number of days and injuries and illness type, annual average number of employees, total hours worked by all employees during a previous year. NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
