29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:    a.   On or about September 9, 2019, a carpentry employee installing framing at the top of the trusses was exposed to a 20 feet fall hazard through the trusses in that he was not protected by a fall protection system.    b.   On or about September 9, 2019, two carpentry employees standing on the top of the wall adjacent to the entrance while installing framing pieces were exposed to a 10 feet fall hazard in that they were not protected by a fall protection system.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:    a.    On or about September 9, 2019, in the center roof trusses, a carpentry employee was exposed to a 20 feet fall hazard while accessing the trusses in that a ladder was not provided to access the roof trusses.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a.    On or about September 9, 2019, a carpentry employee was exposed to a 10 feet fall hazard over the front entrance in that the extension ladder used to access the top of the wall did not extend 3 feet above the wall top plate.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  a.    On or about September 9, 2019, at the front entrance a six-foot fiberglass stepladder that was folded and leaned against a wall was used to climb up to the 10-feet high top plate, exposing an employee to a 5 feet fall hazard from the top of the stepladder.  b.    On or about September 9, 2019, at the center of the house, a six-foot fiberglass stepladder that was folded and leaned against wall bracing frames was used to climb up into the trusses, exposing a carpentry employee to a 5 feet fall hazard from the top of the stepladder.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    a.    On or about September 9, 2019, a carpentry employee operating a circular saw to cut roof sheathing boards was exposed to an eye injury hazard from flying particles in that he was not wearing protective eye glasses.    b.    On or about September 9, 2019, three carpentry employees operating pneumatic nailers while installing truss framing pieces were exposed to an eye injury hazard from flying debris in that they were not wearing protective eye glasses.    Greenday Construction, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1319978, Serious citation number 1, item number 2 and was affirmed as a final order on July 17, 2018, with respect to a workplace located at 5101 Gulf Drive, Panama City Beach, Florida.
