29 CFR 1910.67(c)(2)(v): A body belt was not worn and lanyard was not attached to the boom or basket when working from an aerial lift: On or about 9/6/19 during tree trimming operations, located at 2607 Kimberly Lane in Westlake, Ohio, the employer failed to ensure that employees trimming trees from the CMC Aerial lift, Model S25, serial number S16A1257, were provided and utilized a body belt and lanyard.
