29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    On or about 9/6/19, during commercial masonry tuck pointing operations, located at 33312 Center Ridge Rd in North Ridgeville, Ohio, the employer failed to ensure that employees conducting work from the JLG 450AJ aerial lift, at heights greater than ten feet (10'), were provided and utilized a body belt and lanyard.
