29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503(a)(2)(I) through (viii): a) Millennium Construction Group at 2351 Federal Blvd, Denver, CO 80211: On or before September 10, 2019, the employer did not ensure that each employee who was exposed to fall hazards had been trained by a competent person. Five employees performing re-roofing activities were not using personal fall arrest systems. This condition exposed employees to fall of approximately 35-40 feet.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems: a) Millennium Construction Group at 2351 Federal Blvd, Denver, CO 80211: On or before September 10, 2019, the employer did ensure that each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems. Employees were reroofing a three-story condominium complex without the use of fall protection. This condition exposed employees to fall hazards of approximately 35-40 feet. Millennium Construction Group was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1360229, citation number 1, Item number 2, and was affirmed as a final order on 12/07/2018, with respect to a workplace located at 1625 N. Murray Blvd., Colorado Springs, CO 80915. Millennium Construction Group was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1372541, citation number 1, item number 1 and was affirmed as a final order on 04/25/2019, with respect to a workplace located at 462 Spruce St., Louisville, CO 80027
