Shoop Ave, Wauseon, Ohio 43567: On or about September 10, 2019, the employer did not ensure that a worker operating an Genie Model #S60 aerial lift basket connected a lanyard to the basket while performing tuck pointing adjacent to the back door of the church there by exposing employees to a fall hazard of approximately 10 feet.
