29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  Production area- On or about September 12, 2019 and at times prior, an employee was exposed to a health hazard in that, the employee was allowed to wear a filtering face piece type respirator (disposable dust mask) and was not given the information regarding the use and limitations of the dust mask contained in Appendix D of the respirator standard. The dust mask was used on a voluntary basis.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  (a) Production area- On or about September 12, 2019 and at times prior, employees were exposed to a health hazard in that, the employer had not established or implemented a hazard communication program to include a written program, maintaining safety data sheets, and training regarding the hazards of the chemicals used at the site. Employees were exposed to hazardous substances such as, but not limited to, xylene, toluene, and 2-butoxyethanol.
