29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used: Cousino Restoration of Toledo LLC. worksite located 419 Cherry St., Wauseon, Ohio 43567, in the attic: On or about September 10, 2019, the employer did not ensure employees were using extension cords that were in good repair in that an extension cord had exposed wires at the end of the extension cord near the plug. Extension cord was used to operate hand tools such as but not limited to Milwaukee Orbital Super Sawzall.

Cousino Restoration of Toledo LLC. worksite located 419 Cherry St., Wauseon, Ohio 43567: On or about September 10, 2019, the employer did not ensure employees were protected from falls while making roof repairs on the single story residential structure. The employee was exposed to a fall distance of approximately 10 feet from a steep slope roof.
