29 CFR 1926.501(b)(4): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: (a) Site on floors 2-4 at staircase landing: On or about September 6, 2019, and at times prior, the employer exposed employees to trip hazards in that, of 8" floor holes were not covered while employees were installing wallpaper at the worksite.
