29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s) Location: Front side of existing home @ 176 Fisherville LN. Westport, MA On or about 09/11/19 an employee working between a concrete window well and the house foundation wall had his arm crushed when the excavator bucket slipped off the concrete structure.
