The PIT operator(s) did not receive retraining.     2). On or about November 5, 2019, the employer failed to provide refresher training to the PIT operator(s) involved in an incident in which equipment was damaged. The PIT, JCB (Model No. 506.36F, Serial No. JCB5CD7GVJ2793680) was used to lift a core-straightener into a railroad car when the rigging failed, causing the core-straightener to swing into the windshield of the PIT causing damage. The PIT operator(s) did not receive retraining.
