29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   On or about September 11, 2019, at the northeast corner and south roof edges of a commercial building under construction located at 5610 SE Grouper Avenue in Stuart, Florida, employees were exposed to a fall hazard of approximately 14 feet - 6 inches when performing carpentry work on the roof of the structure with a 3 on 12 pitch while not being protected from falling to the lower level.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   On or about September 11, 2019, on site of a commercial building under construction located at 5610 SE Grouper Avenue in Stuart, Florida, employees were exposed to a fall hazard when performing carpentry work on the roof of the structure prior to being provided with training that would enable them to recognize the hazards associated with falls.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about September 11, 2019, on site of a commercial building under construction located at 5610 SE Grouper Avenue in Stuart, Florida, employees were exposed to a fall hazard of approximately 10 feet when gaining access to and from a roof while using a ladder that was not extended at least 3 feet past the roof edge and was not secured to prevent accidental displacement.
