A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. On or about September 10, 2019 and times prior to, employees did not have a means of egress when spreading gravel inside a trench approximately 9 feet in depth.

29 CFR 1926.701(b): Reinforcing steel. All protruding reinforcing steel, onto and into which employees could fall, were not guarded to eliminate the hazard of impalement. On or about September 10, 2019 and times prior to, employees were exposed to impalement hazard by working next to rebar that was not guarded against impalement.
